PA Manaul Awards (Civilian)

I am trying to track down a template for a "Coast Guard Certificate of Appreciation" that is listed in the CG Public Affairs Manual. Appendix A does not list have the template examples in it. I am trying to give one of these to a civilian boater that helped us out with the MEDEVAC of a child. I need to ensure I have the opening and closing verbage correct, if there is any. Any help is appreciated. I am in the global if you have anything.
